De <\/span><a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/post-covid-19-condition-(long-covid)\"><span style=\"font-weight: 400;\">WHO <\/span><\/a><span style=\"font-weight: 400;\">identificeert bepaalde groepen met een verhoogd risico op post-COVID-19-aandoeningen na SARS-CoV-2-infectie. Dit betreft onder meer vrouwen, ouderen, rokers, mensen met overgewicht of obesitas, personen met bestaande chronische aandoeningen en mensen met een beperking.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Veel mensen ervaren weken of maanden na infectie nog klachten, terwijl anderen nieuwe of verergerde chronische aandoeningen ontwikkelen. Inzicht in deze langetermijneffecten is essentieel voor effectieve, pati\u00ebntgerichte zorg, vooral bij ouderen en mensen met een verhoogd risico op chronische ziekte. Voor zorgprofessionals vereist dit een verschuiving van acute behandeling naar voortdurende monitoring, preventie en geco\u00f6rdineerde zorgplanning.<\/span><\/p>\n<p><b>Wat is Long COVID? Een complex en zich ontwikkelend ziektebeeld<\/b><\/p>\n<p><a href=\"https:\/\/www.nature.com\/articles\/s41467-025-59012-w\"><span style=\"font-weight: 400;\">Long COVID,<\/span><\/a><span style=\"font-weight: 400;\"> ook wel de post-COVID-19- aandoening genoemd, wordt gedefinieerd als het aanhouden of ontstaan van nieuwe symptomen drie maanden na de initi\u00eble SARS-CoV-2-infectie, waarbij deze symptomen minstens twee maanden aanhouden en geen andere verklaring hebben (dit is een WHO-definitie; andere organisaties hanteren licht afwijkende definities). Veelvoorkomende symptomen zijn vermoeidheid, kortademigheid, cognitieve problemen (\u201cbrain fog\u201d), pijn op de borst en slaapstoornissen. Deze klachten kunnen fluctueren en meerdere orgaansystemen tegelijk be\u00efnvloeden. Clinici moeten zich ervan bewust zijn dat de symptomatologie vaak aspecifiek is en een brede, systeemgerichte evaluatie kan vereisen.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">De prevalentie schattingen vari\u00ebren, maar Long COVID lijkt vaker voor te komen bij <\/span><a href=\"https:\/\/journals.plos.org\/Plosmedicine\/article?id=10.1371\/journal.pmed.1004194\"><span style=\"font-weight: 400;\">ouderen<\/span><\/a><span style=\"font-weight: 400;\">, mensen met <\/span><a href=\"https:\/\/academic.oup.com\/ofid\/article\/10\/7\/ofad233\/7150886?login=false&amp;guestAccessKey=\"><span style=\"font-weight: 400;\">bestaande aandoeningen<\/span><\/a><span style=\"font-weight: 400;\"> en pati\u00ebnten die <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41467-023-43661-w\"><span style=\"font-weight: 400;\">een ernstiger beloop <\/span><\/a><span style=\"font-weight: 400;\">van de initi\u00eble infectie hadden. Tegelijkertijd kan het ook optreden bij mensen die een milde of zelfs asymptomatische infectie hadden, waardoor het een breed en onvoorspelbaar ziektebeeld is.<\/span><\/p>\n<p><b>Hoe COVID-19 het risico op chronische ziekten verandert<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Steeds meer bewijs laat zien dat COVID-19 in verband wordt gebracht met het ontstaan of verergeren van <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8220960\/\"><span style=\"font-weight: 400;\">chronische aandoeningen<\/span><\/a><span style=\"font-weight: 400;\">. Zo ontwikkelen sommige pati\u00ebnten <\/span><a href=\"https:\/\/academic.oup.com\/jcem\/article\/108\/11\/e1164\/7174006?login=false&amp;guestAccessKey=\"><span style=\"font-weight: 400;\">nieuw ontstane diabetes<\/span><\/a><span style=\"font-weight: 400;\"> na een infectie, mogelijk door virale effecten op de alvleesklierfunctie of ontstekingsgerelateerde metabole veranderingen. Evenzo zijn <\/span><a href=\"https:\/\/link.springer.com\/article\/10.1007\/s10557-023-07465-w\"><span style=\"font-weight: 400;\">cardiovasculaire complicaties<\/span><\/a><span style=\"font-weight: 400;\"> zoals myocardiale schade, hartritmestoornissen en een verhoogd risico op hartfalen waargenomen, zelfs na herstel van de acute fase van de ziekte. Daarom is voortdurende monitoring van cardiometabole risicofactoren een belangrijk onderdeel van post-COVID-zorg.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Deze associaties onderstrepen de noodzaak om COVID-19 niet alleen te zien als een acute respiratoire infectie, maar ook als een aandoening met systemische en langdurige gezondheidseffecten. Bij pati\u00ebnten die al leven met chronische ziekten kan COVID-19 het ziektebeloop versnellen of de behandeling complexer maken.<\/span><\/p>\n<p><b>Ouderen na COVID-19: herstel, achteruitgang en veerkracht<\/b><\/p>\n<p><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11377967\/\"><span style=\"font-weight: 400;\">Ouderen <\/span><\/a><span style=\"font-weight: 400;\">zijn onevenredig zwaar getroffen door zowel de acute als de langetermijneffecten van COVID-19. <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lanhl\/article\/PIIS2666-7568(22)00245-8\/fulltext\"><span style=\"font-weight: 400;\">Mensen van 65 jaar en ouder <\/span><\/a><span style=\"font-weight: 400;\">hebben een grotere kans om langdurige COVID-19-symptomen en complicaties te ontwikkelen dan jongere volwassenen. Daarnaast kan COVID-19 bestaande chronische aandoeningen, zoals cardiovasculaire en respiratoire ziekten en neurodegeneratieve aandoeningen, zowel uitlokken als verergeren.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Naast medische complicaties ervaren veel ouderen functionele achteruitgang, waaronder verminderde mobiliteit, spierzwakte en verlies van zelfstandigheid.<\/span><a href=\"https:\/\/www.cambridge.org\/core\/journals\/international-psychogeriatrics\/article\/prevalence-of-loneliness-and-social-isolation-among-older-adults-during-the-covid19-pandemic-a-systematic-review-and-metaanalysis\/8B1F4AE61E9CD44028B7EAFA1313C876\"><span style=\"font-weight: 400;\"> Sociale isolatie <\/span><\/a><span style=\"font-weight: 400;\">tijdens en na de ziekte kan daarnaast bijdragen aan cognitieve achteruitgang en psychische klachten zoals depressie en angst. <\/span><a href=\"https:\/\/link.springer.com\/article\/10.1186\/s12877-023-03757-w\"><span style=\"font-weight: 400;\">De kwaliteit van leven<\/span><\/a><span style=\"font-weight: 400;\"> wordt vaak aanzienlijk be\u00efnvloed. Zelfs na ogenschijnlijk herstel kunnen oudere pati\u00ebnten moeite hebben om terug te keren naar hun eerdere functieniveau, waardoor extra ondersteuning nodig is van zorgverleners, mantelzorgers en maatschappelijke diensten. Routinematige beoordeling van functionele status, mentale gezondheid en sociale ondersteuning is daarom bijzonder belangrijk in deze populatie.<\/span><\/p>\n<p><b>Waarom Long COVID een teamgerichte, langdurige zorgaanpak vereist<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Gezien de brede variatie aan post-COVID-complicaties is een multidisciplinaire benadering van zorg essentieel. Dit kan input omvatten van huisartsen, specialisten (zoals cardiologen of endocrinologen), fysiotherapeuten, psychologen en maatschappelijk werkers. Regelmatige follow-up maakt vroege herkenning van nieuwe of verergerende aandoeningen mogelijk en ondersteunt tijdige interventie. Het opzetten van duidelijke verwijsroutes en communicatielijnen tussen zorgverleners kan de continu\u00efteit van zorg verbeteren.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Belangrijk is dat de zorg ge\u00efndividualiseerd moet worden. Pati\u00ebnten met Long COVID presenteren zich vaak met uiteenlopende en overlappende symptomen, en hun hersteltrajecten kunnen sterk verschillen. Het integreren van Long COVID-aspecten in bestaande zorgplannen voor chronische ziekten kan de uitkomsten verbeteren en het risico op verdere complicaties verminderen. Gedeelde besluitvorming en het gezamenlijk formuleren van doelen kunnen helpen om de zorg beter af te stemmen op de prioriteiten en mogelijkheden van de pati\u00ebnt.<\/span><\/p>\n<p><b>Vooruitblik: het versterken van bewustwording, vertrouwen en continu\u00efteit in de praktijk<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Zorgprofessionals spelen een sleutelrol in het herkennen en aanpakken van de langetermijneffecten van COVID-19. Het vergroten van bewustzijn bij pati\u00ebnten, het erkennen van hun klachten en het bieden van duidelijke richtlijnen kan helpen om onzekerheid te verminderen en de betrokkenheid bij de zorg te verbeteren. Het opbouwen van vertrouwen in het bespreken van Long COVID met pati\u00ebnten is een belangrijk onderdeel van de klinische praktijk.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Terwijl het onderzoek zich blijft ontwikkelen, is het belangrijk om goed ge\u00efnformeerd en flexibel te blijven. De langetermijnlast van COVID-19 zal waarschijnlijk aanhouden, waardoor het een belangrijk onderdeel wordt van chronische ziektezorg in de komende jaren. Voortdurende nascholing van zorgprofessionals (inclusief deze training) en het integreren van nieuwe wetenschappelijke inzichten in de praktijk zullen essentieel zijn om optimale pati\u00ebntuitkomsten te bereiken.<\/span><\/p>\n<p><b>Praktische aanbevelingen<\/b><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Vraag proactief naar persisterende klachten<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Vraag tijdens routineconsulten actief naar aanhoudende symptomen na COVID-19, ook als de initi\u00eble infectie mild was. Vermoeidheid, kortademigheid en cognitieve veranderingen worden vaak ondergerapporteerd. Gerichte vragen maken vroege herkenning, tijdige beoordeling en zo nodig verwijzing mogelijk.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Screen op nieuwe of verergerende chronische aandoeningen<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Let op signalen van nieuw ontstane diabetes, cardiovasculaire complicaties of verslechtering van bestaande aandoeningen. Dit kan onder meer het controleren van bloedglucose, bloeddruk en cardiale symptomen omvatten. Het integreren van deze screening in post-COVID follow-up kan helpen om complicaties vroegtijdig op te sporen.<\/p>\n<p><\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Ondersteun functioneel herstel bij ouderen<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Stimuleer een geleidelijke terugkeer naar fysieke activiteit, afgestemd op de belastbaarheid van de pati\u00ebnt. Revalidatie en activiteitsopbouw moeten ge\u00efndividualiseerd worden, omdat sommige pati\u00ebnten klachtenverergering ervaren na overbelasting. Verwijzing naar fysiotherapie of revalidatiediensten kan mobiliteit, kracht en zelfstandigheid ondersteunen. Overweeg ook cognitieve en mentale gezondheidscreening, vooral bij pati\u00ebnten die langdurige isolatie hebben doorgemaakt.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Hanteer een multidisciplinaire aanpak<br \/>\n<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Co\u00f6rdineer zorg waar mogelijk tussen verschillende disciplines. Pati\u00ebnten met complexe of aanhoudende klachten kunnen baat hebben bij gecombineerde input van medische, paramedische en psychosociale diensten. Duidelijke communicatie tussen zorgverleners is essentieel om gefragmenteerde zorg te voorkomen.<br \/>\n<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Bied geruststelling en stel realistische verwachtingen<\/b><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Herstel van Long COVID kan traag en wisselend verlopen. Erken zorgen van pati\u00ebnten, valideer hun ervaringen en geef duidelijke informatie over het te verwachten beloop.
