Per le persone che convivono con condizioni croniche, la sfida non \u00e8 solo gestire i sintomi ma <\/span><a href=\"https:\/\/www.annualreviews.org\/content\/journals\/10.1146\/annurev-psych-010416-044014;jsessionid=N5r4qZAzsSMU4ZQD7LLAmHH-eAvt63vdFXYAmhdF.annurevlive-10-241-10-103\"><span style=\"font-weight: 400;\">adattarsi ad una vita diversa<\/span><\/a><span style=\"font-weight: 400;\">. Quando questo processo di adattamento \u00e8 perturbato, pu\u00f2 contribuire all\u2019insorgenza di <\/span><a href=\"https:\/\/karger.com\/pps\/article-pdf\/92\/4\/227\/4001489\/000533263.pdf\"><span style=\"font-weight: 400;\">ansia, depressione, ad una salute fisica compromessa<\/span><\/a><span style=\"font-weight: 400;\"> e ad un <\/span><a href=\"https:\/\/qualitysafety.bmj.com\/content\/qhc\/27\/12\/989.full.pdf\"><span style=\"font-weight: 400;\">uso maggiore dei servizi sanitari<\/span><\/a><span style=\"font-weight: 400;\">. Le stime di come le persone si adattano alle condizioni croniche variano ampiamente, a seconda di come viene misurato l\u2019adattamento. Le stime di un cattivo adattamento oscillano tra il <\/span><a href=\"https:\/\/dergipark.org.tr\/en\/download\/article-file\/1226414\"><span style=\"font-weight: 400;\">16.9<\/span><\/a><span style=\"font-weight: 400;\"> e il <\/span><a href=\"https:\/\/www.mdpi.com\/2227-9059\/10\/12\/3231\"><span style=\"font-weight: 400;\">62%<\/span><\/a><span style=\"font-weight: 400;\">. Per quanto riguarda invece un buon adattamento la stima va dal <\/span><a href=\"https:\/\/qualitysafety.bmj.com\/content\/qhc\/27\/12\/989.full.pdf\"><span style=\"font-weight: 400;\">13<\/span><\/a><span style=\"font-weight: 400;\"> al <\/span><a href=\"https:\/\/link.springer.com\/content\/pdf\/10.1186\/s40359-025-03115-y.pdf\"><span style=\"font-weight: 400;\">36.3%<\/span><\/a><span style=\"font-weight: 400;\">. Su scala globale, pi\u00f9 di <\/span><a href=\"https:\/\/www.thelancet.com\/action\/showPdf?pii=S2589-5370%2823%2900037-8\"><span style=\"font-weight: 400;\">1 miliardo di persone convive con pi\u00f9 condizioni croniche<\/span><\/a><span style=\"font-weight: 400;\">. Se fino a due terzi di questi sperimenta difficolt\u00e0 di adattamento, questo indica che c\u2019\u00e8 un\u2019urgenza nel migliorare il supporto all\u2019adattamento a condizioni croniche.<\/span><\/p>\n<p><!--more--><\/p>\n<p><span style=\"font-weight: 400;\">L\u2019adattamento non si limita all\u2019aderenza a consigli medici, n\u00e9 riguarda solamente i comportamenti salutari. <\/span><a href=\"https:\/\/www.thelancet.com\/action\/showPdf?pii=S2589-5370%2823%2900037-8\"><span style=\"font-weight: 400;\">Il Modello Integrato di Adattamento a Sfide Continue [precedentemente \u201ca Condizioni Croniche\u201d] (IMACC)<\/span><\/a><span style=\"font-weight: 400;\"> \u00e8 una teoria che riguarda il processo biopsicosociale di adattamento, per come viene sperimentato dalla persona. IMACC fu sviluppato attraverso uno <\/span><a href=\"https:\/\/journals.sagepub.com\/doi\/pdf\/10.1177\/1359105316664131?casa_token=LLPf18ZAce8AAAAA:ccMmLiXCDQj7ncUmEanGjCKpDFRZek7Iu6ZfR_-WtJDWnJzsqwoDO_uAZE5kzLvwF0Vg7RKTLCKN\"><span style=\"font-weight: 400;\">studio sul diabete di tipo 2<\/span><\/a><span style=\"font-weight: 400;\"> e successivamente ne \u00e8 stata <\/span><a href=\"https:\/\/journals.sagepub.com\/doi\/pdf\/10.1177\/2055102919884293\"><span style=\"font-weight: 400;\">verificata l\u2019adattabilit\u00e0 anche all\u2019epilessia con esordio in et\u00e0 adulta<\/span><\/a><span style=\"font-weight: 400;\">. Usare IMACC in maniera collaborativa pu\u00f2 offrire uno sguardo diretto del processo di adattamento della persona, come verr\u00e0 descritto sotto.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2590291125003481\"><span style=\"font-weight: 400;\">L\u2019adattamento \u00e8 un processo normale e che si svolge lungo tutta la vita<\/span><\/a><span style=\"font-weight: 400;\">; quando la vita cambia, noi cambiamo con essa. Tuttavia, pu\u00f2 essere difficile adattarsi ai cambiamenti di vita negativi. IMACC suggerisce che per rendere la vita di tutti i giorni il pi\u00f9 possibile funzionale, pu\u00f2 essere necessario cambiare i pensieri ed i comportamenti che non sono pi\u00f9 d\u2019aiuto nelle nuove condizioni. Questi cambiamenti sono chiamati <\/span><i><span style=\"font-weight: 400;\">compiti di adattamento<\/span><\/i><span style=\"font-weight: 400;\">. La risoluzione efficace di questi porta ad un miglior adattamento, ad una vita quotidiana pi\u00f9 funzionale ed una riformulazione positiva della propria identit\u00e0. Le difficolt\u00e0 di adattamento possono essere causate da <\/span><i><span style=\"font-weight: 400;\">ostacoli<\/span><\/i><span style=\"font-weight: 400;\"> che si frappongono al cambiamento, che potrebbero bloccare il processo di adattamento e, con il tempo, portare ad uno scarso adattamento, un deterioramento della salute fisica e mentale e una riformulazione negativa della propria identit\u00e0.\u00a0<\/span><\/p>\n<p><strong>Come \u00e8 strutturato IMACC<\/strong><\/p>\n<p><span style=\"font-weight: 400;\">IMACC consiste in tre livelli interattivi:\u00a0<\/span><\/p>\n<p><strong>Livello 1: Personalit\u00e0 e cambiamenti di vita<\/strong><span style=\"font-weight: 400;\"> riguarda la personalit\u00e0, le credenze ed i comportamenti passati. Il focus \u00e8 sugli aspetti che potrebbero impattare sul processo di adattamento, sia come <\/span><i><span style=\"font-weight: 400;\">ostacoli<\/span><\/i><span style=\"font-weight: 400;\"> al processo che come <\/span><i><span style=\"font-weight: 400;\">risorse<\/span><\/i><span style=\"font-weight: 400;\"> che possono supportare l\u2019adattamento. In aggiunta, <\/span><i><span style=\"font-weight: 400;\">esperienze determinanti <\/span><\/i><span style=\"font-weight: 400;\">che avvengono in relazione a eventi di vita negativi causando il bisogno di adattamento, potrebbero a loro volta avere un impatto sul processo di adattamento, sia negativamente (per es. incidenti nella cura) che positivamente (per es. supporto).\u00a0<\/span><\/p>\n<p><strong>Livello 2: Ciclo di adattamento continuo<\/strong> <span style=\"font-weight: 400;\">consiste in cinque aree con compiti di adattamento. Quando si fronteggia una condizione cronica o disabilit\u00e0, la persona inizia <\/span><b>Facendo il punto della situazione<\/b><span style=\"font-weight: 400;\">. Si elaborano reazioni emotive iniziali come shock o rabbia mentre si riflette su cosa dovrebbe cambiare per far s\u00ec che la vita torni a funzionare. Questo implica la costituzione di una motivazione all\u2019adattamento.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">La persona procede poi nell\u2019<\/span><b>Imparare<\/b><span style=\"font-weight: 400;\">, acquisire <\/span><i><span style=\"font-weight: 400;\">conoscenza<\/span><\/i><span style=\"font-weight: 400;\"> sulla propria condizione e sulle le opzioni disponibili, cos\u00ec come <\/span><i><span style=\"font-weight: 400;\">competenze<\/span><\/i><span style=\"font-weight: 400;\"> per l\u2019autogestione e il coping. Ci\u00f2 implica tipicamente sviluppare nuovi <\/span><i><span style=\"font-weight: 400;\">atteggiamenti<\/span><\/i><span style=\"font-weight: 400;\"> verso la vita e gli altri. Il <\/span><b>Contesto<\/b><span style=\"font-weight: 400;\"> della persona gioca un ruolo importante. Famiglia, lavoro, professionisti della salute e l\u2019ambiente pi\u00f9 ampio influenzano l\u2019adattamento, cos\u00ec come la percezione della persona del suo contesto e dei propri ruoli di vita.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Mentre i processi di adattamento avanzano, la persona ha bisogno di impegnarsi nel <\/span><b>Lasciare Andare<\/b><span style=\"font-weight: 400;\">, elaborando perdite legate al funzionamento passato o a sogni futuri e piani che non sono pi\u00f9 realizzabili. Potrebbe manifestarsi la necessit\u00e0 di abbandonare atteggiamenti o comportamenti che non funzionano pi\u00f9 nella nuova situazione. Con il tempo, per ogni cambiamento realizzato, la persona potrebbe <\/span><b>Accettare e Integrare <\/b><span style=\"font-weight: 400;\">il nuovo modo di vivere, integrando e mantenendo i nuovi atteggiamenti e comportamenti nella vita quotidiana.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Significativamente, per ogni problema che richiede un cambiamento, la persona si muove attraverso il <\/span><b>Ciclo di adattamento continuo<\/b><span style=\"font-weight: 400;\">, il che significa che molte aree possono essere attive contemporaneamente.\u00a0<\/span><\/p>\n<p><strong>Livello 3: Ciclo di mantenimento<\/strong><span style=\"font-weight: 400;\"> mostra cosa accade nei momenti sfidanti. \u00c8 un classico ciclo di mantenimento cognitivo-comportamentale con una differenza: le cognizioni sono caratterizzate da un conflitto tra <\/span><i><span style=\"font-weight: 400;\">Fare il punto<\/span><\/i><span style=\"font-weight: 400;\"> o <\/span><i><span style=\"font-weight: 400;\">Imparare<\/span><\/i><span style=\"font-weight: 400;\"> (il che dipende da a che punto del processo di adattamento la persona si \u00e8 bloccata), <\/span><i><span style=\"font-weight: 400;\">Lasciare Andare<\/span><\/i><span style=\"font-weight: 400;\"> ed il <\/span><i><span style=\"font-weight: 400;\">Contesto<\/span><\/i><span style=\"font-weight: 400;\">. Per meglio comprendere, un esempio (stereo-)tipico \u00e8 una donna, che fa fatica ad <\/span><i><span style=\"font-weight: 400;\">imparare<\/span><\/i><span style=\"font-weight: 400;\"> ad avere un maggiore focus sul prendersi cura di s\u00e9, perch\u00e9 non riesce a <\/span><i><span style=\"font-weight: 400;\">lasciare andare<\/span><\/i><span style=\"font-weight: 400;\"> il suo ruolo di persona che cucina e pulisce per paura di essere una \u201ccattiva madre\u201d o una \u201ccattiva moglie\u201d. Mantenere le sue relazioni strette (<\/span><i><span style=\"font-weight: 400;\">contesto<\/span><\/i><span style=\"font-weight: 400;\"> sociale) viene percepito come pi\u00f9 importante che prendersi cura di s\u00e9 (<\/span><i><span style=\"font-weight: 400;\">atteggiamento<\/span><\/i><span style=\"font-weight: 400;\"> implicito).<\/span><\/p>\n<p><strong>Raccomandazioni pratiche<\/strong><\/p>\n<p><strong>Identificare un obiettivo significativo<\/strong><span style=\"font-weight: 400;\">: Supportare la riflessione circa quali cambiamenti nella vita quotidiana della persona possono apportare una differenza significativa (per es. cambiamenti nei comportamenti salutari). Discutere su come la persona pu\u00f2 integrare questo cambiamento nella sua routine quotidiana. Essere specifici sui dettagli (tempo, posto, ecc.) semplifica l\u2019inizio del processo. Chiedere cosa significherebbe per loro attuare questo cambiamento \u2013 e cosa potrebbe avvenire se la persona NON cambiasse. Avere obiettivi significativi contribuisce alla motivazione.\u00a0<\/span><\/p>\n<p><strong>Lavorare per superare gli ostacoli<\/strong><span style=\"font-weight: 400;\">: Esaminare i tentativi di implementare il cambiamento e normalizzare qualsiasi difficolt\u00e0 sia emersa. Discutere un recente tentativo di cambiamento e stare attenti a valori e atteggiamenti impliciti (es. \u201cDevo aiutare gli altri prima che me stesso\u201d o \u201cDevo finire il compito prima di riposarmi\u201d).\u00a0<\/span><\/p>\n<p><strong>Considerare il ruolo degli altri<\/strong><span style=\"font-weight: 400;\">: Chiedere alla persona che impatto crede che il cambiamento possa avere sulle relazioni strette. Ad esempio, se un bisogno di migliorare la cura di s\u00e9 significa fare meno per gli altri, quali crede che possano essere le conseguenze? Cosa potrebbe significare per la sua identit\u00e0 (es. cambiamento di ruoli di vita). Cosa crede che gli altri possano pensare, dire o fare?<\/span><\/p>\n<p><strong>Supportare le negoziazioni circa il cambiamento<\/strong><b>: <\/b><span style=\"font-weight: 400;\">Di cosa la persona potrebbe sentire il bisogno di parlare con gli altri? Potrebbe aver bisogno di chiedere cosa significa per gli altri (invece di basarsi sulle sue assunzioni). Potrebbe aver bisogno di negoziare cambiamenti nella routine quotidiana o migliorare le proprie competenze nel comunicare i propri bisogni (es. livelli di dolore fluttuanti \u2013 cosa gli altri si aspettano che la persona sar\u00e0 in grado di fare oggi).\u00a0<\/span><\/p>\n<p><strong>Migliorare la<\/strong> <a href=\"https:\/\/www.tandfonline.com\/doi\/pdf\/10.1080\/09593985.2018.1434578?casa_token=U3mxijFzTCQAAAAA:Ly01NghmTu83gDXguppguI2Bv1qPS_0UgJPHg9AUUwjH_mcSJwnIFDBbQx9lQFDk2z4x5t_cTlKQ\"><b>consapevolezza corporea di base<\/b><\/a><span style=\"font-weight: 400;\">: Supportare la persona nell\u2019imparare a comprendere i sintomi corporei e le sensazioni, cosa vogliono dire e come usarli per guidare la cura di s\u00e9.
