Ti, kte\u0159\u00ed tyto<a href=\"https:\/\/www.thelancet.com\/journals\/lanres\/article\/PIIS2213-2600(21)00383-0\/fulltext\"> p\u0159\u00edznaky<\/a> pro\u017e\u00edvaj\u00ed, to naz\u00fdvaj\u00ed \u201eLong-COVID\u201c.<\/p>\n<p><!--more--><\/p>\n<p><strong>Moje zku\u0161enost\u2026<\/strong><\/p>\n<p>Pr\u00e1ce na Long-COVID klinice mi uk\u00e1zala, \u017ee lid\u00e9 maj\u00ed probl\u00e9m p\u0159ijmout sv\u00e9 novou situaci (a n\u00e1sledky t\u011bchto p\u0159\u00edznak\u016f), co\u017e jim br\u00e1n\u00ed v\u00a0adaptaci a p\u0159ijmut\u00ed nov\u00fdch strategi\u00edch sebe\u0159\u00edzen\u00ed. Zam\u011b\u0159uj\u00ed se na to, co dok\u00e1zali d\u011blat p\u0159ed \u201eLong-COVIDem\u201c a jak se jejich \u017eivot zhor\u0161il od diagn\u00f3zy. To nazna\u010duje vyh\u00fdbav\u00e9 chov\u00e1n\u00ed (tj. pokus nebo touhu potla\u010dit necht\u011bn\u00e9 vnit\u0159n\u00ed pro\u017eitky, jako jsou emoce, my\u0161lenky, vzpom\u00ednky a t\u011blesn\u00e9 pocity) a tendenci lid\u00ed soust\u0159edit se na formu p\u0159\u00edznak\u016f nam\u00edsto funkce p\u0159\u00edznak\u016f (tj. t\u011blesn\u00e9ho sign\u00e1lu k zm\u011bn\u011b chov\u00e1n\u00ed). Ukazuje to, \u017ee lid\u00e9 nejsou otev\u0159eni nov\u00fdm zku\u0161enostem a vyh\u00fdbaj\u00ed se \u017eit\u00ed v p\u0159\u00edtomn\u00e9m okam\u017eiku.<\/p>\n<p>Nav\u00edc, <a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2021.03.22.21254057v2\">v\u00edce ne\u017e 25 % lid\u00ed<\/a>, kte\u0159\u00ed byli hospitalizov\u00e1ni s COVID-19, m\u00e1 trval\u00e9 p\u0159\u00edznaky deprese i 6 m\u011bs\u00edc\u016f pot\u00e9, co byli propu\u0161t\u011bni z nemocnice. Jeliko\u017e je deprese spojena s\u00a0<a href=\"https:\/\/www.mindwell-leeds.org.uk\/myself\/exploring-your-mental-health\/depression\/what-keeps-depression-going\/\">vyh\u00fdbav\u00fdm chov\u00e1n\u00edm<\/a> (tj. nedostatku energie zapojen\u00ed se do situace), to ukazuje na to, \u00a0\u017ee lid\u00e9 se mo\u017en\u00e1 neanga\u017euj\u00ed v\u00a0p\u0159\u00ednosn\u00fdch aktivit\u00e1ch.<\/p>\n<p><strong>Role psychick\u00e9 flexibility u Long-COVID<\/strong><\/p>\n<p>Psychick\u00e1 flexibilita p\u0159edstavuje mechanismus p\u016fsoben\u00ed <a href=\"https:\/\/www.ijpsy.com\/volumen10\/num1\/256.html\">Terapie p\u0159ijet\u00ed (ACT).<\/a> Zahrnuje t\u0159i pil\u00ed\u0159e terapie: (1) podporov\u00e1n\u00ed otev\u0159enosti ke zku\u0161enostem, (2) uv\u011bdom\u011bn\u00ed si p\u0159\u00edtomn\u00e9ho okam\u017eiku a (3) zapojen\u00ed se do p\u0159\u00ednosn\u00fdch aktivit. Jak bylo uvedeno v\u00fd\u0161e, u lid\u00ed \u017eij\u00edc\u00edch s\u00a0Long-COVID se tyto pil\u00ed\u0159e zdaj\u00ed b\u00fdt nevyv\u00e1\u017een\u00e9, a proto je jejich \u0159e\u0161en\u00ed d\u016fle\u017eit\u00e9. ACT p\u0159i Long-COVID ji\u017e <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34931160\/\">dosahuje pokrok\u016f<\/a>, \u00fa\u010dastn\u00edci hl\u00e1sili zlep\u0161en\u00fd wellbeing, st\u00e1le pro\u017e\u00edvali \u00fazkost spojenou s COVID, ale c\u00edtili se schopn\u011bj\u0161\u00ed zvl\u00e1dat obecnou psychickou z\u00e1t\u011b\u017e (jako je \u00fazkost).<\/p>\n<p><strong>Pou\u017eit\u00ed ACT na klinice pro Long-COVID: P\u0159edstavuji Jo<\/strong><\/p>\n<p>Kdy\u017e jsem se poprv\u00e9 setkal s Jo, za\u017e\u00edval\/a pravideln\u00e9 relapsy p\u0159\u00edznak\u016f. Za \u00fa\u010delem uchov\u00e1n\u00ed anonymity p\u0159edstavuje Jo \u0161k\u00e1lu lid\u00ed, se kter\u00fdmi jsem pracoval na klinice pro Long-COVID. Jejich nejv\u00edce zat\u011b\u017euj\u00edc\u00edmi p\u0159\u00edznaky byla du\u0161nost a \u00fanava, co\u017e znesnad\u0148ovalo b\u011b\u017en\u00e9 denn\u00ed aktivity. Jo se b\u00e1l(a) t\u011bchto atak p\u0159\u00edznak\u016f a aktivn\u011b se sna\u017eil(a) vyhnout v\u0161emu, co by je mohlo zhor\u0161it (tj. vyh\u00fdbav\u00e9 chov\u00e1n\u00ed, vyh\u00fdb\u00e1n\u00ed se p\u0159\u00edtomn\u00e9mu okam\u017eiku). V\u011bnoval(a) m\u00e1lo \u010dasu \u010dinnostem, kter\u00e9 mu(j\u00ed) p\u0159in\u00e1\u0161ely radost a smysl (tj. vyh\u00fdb\u00e1n\u00ed se zapojen\u00ed do hodnotn\u00fdch akc\u00ed).<\/p>\n<p>V\u00a0na\u0161ich spole\u010dn\u00fdch setk\u00e1n\u00ed jsme se v\u011bnovali v\u0161em komponent\u016fm ACT:<\/p>\n<ul>\n<li><strong>Kognitivn\u00ed defuze<\/strong> (tj. v\u0161\u00edm\u00e1n\u00ed si my\u0161lenek sp\u00ed\u0161e ne\u017e no\u0159en\u00ed se do nich, nebo je p\u0159ijmout, a nechat my\u0161lenky p\u0159ich\u00e1zet a odch\u00e1zet m\u00edsto toho, aby se v my\u0161lenk\u00e1ch setrvalo) &#8211; k odli\u0161en\u00ed mezi my\u0161lenkami\/pocity a skute\u010dn\u00fdmi z\u00e1\u017eitky (nejsou nutn\u011b toto\u017en\u00e9!).<\/li>\n<li><strong>Kontakt s p\u0159\u00edtomn\u00fdm okam\u017eikem<\/strong> &#8211; vyu\u017eit\u00ed strategi\u00ed mindfulness k zako\u0159en\u011bn\u00ed se v jejich smyslov\u00fdch z\u00e1\u017eitc\u00edch (tj. uv\u011bdom\u011bn\u00ed p\u0159\u00edtomn\u00e9ho okam\u017eiku).<\/li>\n<li><strong>Zkoum\u00e1n\u00ed hodnot<\/strong> \u2013 identifikace oblast\u00ed \u017eivota, kterou jsou pro n\u011b nejd\u016fle\u017eit\u011bj\u0161\u00ed.<\/li>\n<li><strong>P\u0159ijmout z\u00e1vazek k \u010dinnosti<\/strong> &#8211; stanovit chov\u00e1n\u00ed sm\u011b\u0159uj\u00edc\u00ed k c\u00edl\u016fm, kter\u00e9 jsou v souladu s jejich hodnotami.<\/li>\n<li><strong>J\u00e1 jako kontext<\/strong> &#8211; nav\u00e1zat kontakt se sv\u00fdm &#8220;pozoruj\u00edc\u00edm j\u00e1&#8221; a vid\u011bt sv\u00e9 my\u0161lenky z d\u00e1lky (tj. Z v\u00edce objektivn\u00edho pohledu) bez toho, aby se do nich zamotali.<\/li>\n<li><strong>P\u0159ijet\u00ed <\/strong>&#8211; p\u0159estat bojovat s t\u00edm, co je mimo jejich kontrolu, a zam\u011b\u0159it se na to, co je pod jejich kontrolou (tj. jejich chov\u00e1n\u00ed).<\/li>\n<\/ul>\n<p>Na druh\u00e9m sezen\u00ed si Jo stanovil(a) dva c\u00edle, kter\u00e9 byly v souladu s jej\u00edmi hlavn\u00edmi hodnotami. Pou\u017eil jsem <a href=\"https:\/\/motivationalinterviewing.org\/understanding-motivational-interviewing\">techniky motiva\u010dn\u00edho rozhovoru<\/a>, abych j\u00ed pomohl <a href=\"https:\/\/www.mindtools.com\/a4wo118\/smart-goals\">stanovit SMART c\u00edl<\/a> a pl\u00e1ny &#8220;<a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/10463283.2020.1808936https:\/www.tandfonline.com\/doi\/full\/10.1080\/10463283.2020.1808936\">kdy\u017e-tak&#8221;<\/a> (if-then) k p\u0159edv\u00edd\u00e1n\u00ed jak\u00fdchkoli obav, kter\u00e9 by mohla m\u00edt.<\/p>\n<p>O \u010dty\u0159i t\u00fddny pozd\u011bji mi s velkou hrdost\u00ed sd\u011blil(a), \u017ee dos\u00e1hl(a) sv\u00fdch c\u00edl\u016f. Tak\u00e9 za\u010dlenila n\u011bkolik cvi\u010den\u00ed mindfulness \u00a0do sv\u00e9ho ka\u017edodenn\u00edho \u017eivota (nap\u0159. cvi\u010den\u00ed se zplo\u0161t\u011bl\u00fdm listem na proud\u00edc\u00ed \u0159ece, den\u00edk vd\u011b\u010dnosti a 5 minut j\u00f3gy). Za\u017eil(a) zv\u00fd\u0161enou psychickou flexibilitu (tj. kdy\u017e je osoba psychologicky flexibiln\u00ed, rozhoduje se na z\u00e1klad\u011b sv\u00fdch hodnot a dlouhodob\u00fdch p\u0159esv\u011bd\u010den\u00ed sp\u00ed\u0161e ne\u017e na z\u00e1klad\u011b okam\u017eit\u00fdch kr\u00e1tkodob\u00fdch emoc\u00ed, kter\u00e9 pr\u00e1v\u011b pro\u017e\u00edv\u00e1), sn\u00ed\u017een\u00ed p\u0159\u00edznak\u016f deprese a zv\u00fd\u0161enou d\u016fv\u011bru v samostatnou seberegulaci mimo sezen\u00ed.<\/p>\n<p><strong>Doporu\u010den\u00ed pro pracovn\u00edky ve zdravotnictv\u00ed pracuj\u00edc\u00ed s lidmi s Long-COVID:<\/strong><\/p>\n<ol>\n<li><strong>Nezapom\u00ednejte na d\u016fle\u017eitost biopsychosoci\u00e1ln\u00ed anamn\u00e9zy &#8211; <\/strong>skute\u010dn\u00e9 porozum\u011bn\u00ed slo\u017eitosti p\u0159\u00edznak\u016f dlouh\u00e9ho pr\u016fb\u011bhu COVID u jednotlivce a d\u016fsledk\u016f t\u011bchto p\u0159\u00edznak\u016f je kl\u00ed\u010dov\u00e9 pro identifikaci strategi\u00ed p\u00e9\u010de orientovan\u00e9 na pacienta. Nebojte se kl\u00e1st ot\u00e1zky jako: &#8220;S jak\u00fdm p\u0159\u00edznakem m\u00e1te nejv\u011bt\u0161\u00ed pot\u00ed\u017ee?&#8221;; &#8220;Jak\u00fd je dopad va\u0161\u00ed du\u0161nosti\/\u00fanavy\/kognitivn\u00edch poruch&#8230; na v\u00e1\u0161 \u017eivot?&#8221;; &#8220;Jak\u00e9 my\u0161lenky se v\u00e1m hon\u00ed hlavou, kdy\u017e pro\u017e\u00edv\u00e1te du\u0161nost\/\u00fanavu\/kognitivn\u00ed obt\u00ed\u017ee?&#8221;<\/li>\n<li><strong>Zva\u017ete zm\u011b\u0159en psychick\u00e9 flexibility &#8211;<\/strong> komplexn\u00ed hodnocen\u00ed proces\u016f Terapie p\u0159ijet\u00ed (<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S2212144716300229\">CompACT<\/a>) je rychl\u00fd a ov\u011b\u0159en\u00fd sebeposuzovac\u00ed zp\u016fsob m\u011b\u0159en\u00ed psychick\u00e9 flexibility.
