{"id":1123,"date":"2019-01-07T23:58:43","date_gmt":"2019-01-07T23:58:43","guid":{"rendered":"http:\/\/ehps.easyall.org\/php\/?p=1123"},"modified":"2025-11-04T14:33:59","modified_gmt":"2025-11-04T14:33:59","slug":"patient-talk-what-the-doctor-clearly-says-and-the-patient-clearly-does-not-understand","status":"publish","type":"post","link":"https:\/\/practicalhealthpsychology.com\/cz\/2019\/01\/patient-talk-what-the-doctor-clearly-says-and-the-patient-clearly-does-not-understand\/","title":{"rendered":"Jak mluvit s pacienty: kdy\u017e l\u00e9ka\u0159 hovo\u0159\u00ed jasn\u011b, ale pacient p\u0159esto nerozum\u00ed"},"content":{"rendered":"<p><strong>Anne Marie Plass, L\u00e9ka\u0159sk\u00e9 centrum Univerzity v G\u00f6ttingenu, N\u011bmecko<\/strong><\/p>\n<p><em>Ned\u00e1vno si mi post\u011b\u017eovala kolegyn\u011b dermatolo\u017eka, kter\u00e1 se ve fakultn\u00ed nemocnici specializuje na psori\u00e1zu (chronick\u00e9 onemocn\u011bn\u00ed k\u016f\u017ee), kolik z\u00a0jej\u00edch pacient\u016f nedodr\u017euje spr\u00e1vn\u011b l\u00e9\u010dbu, i kdy\u017e si spole\u010dn\u011b nastavili c\u00edle a spole\u010dn\u011b se rozhodli pro formu l\u00e9\u010dby.<\/em><!--more--><\/p>\n<p>Spole\u010dn\u00e9 rozhodov\u00e1n\u00ed definujeme jako \u201ep\u0159\u00edstup, kdy spolu klini\u010dt\u00ed pracovn\u00edci a pacienti stoj\u00edc\u00ed p\u0159ed d\u016fle\u017eit\u00fdm <a href=\"https:\/\/www.bmj.com\/content\/341\/bmj.c5146.long\">rozhodnut\u00edm<\/a> sd\u00edlej\u00ed nejlep\u0161\u00ed dostupn\u00e9 informace\u201c, a to obvykle v\u00a0situac\u00edch, kdy jsou p\u0159i l\u00e9\u010db\u011b dostupn\u00e9 dv\u011b nebo t\u0159i mo\u017enosti, nebo kdy\u017e je zapojeno v\u00edce osob.<\/p>\n<p>Jak pacienti, tak l\u00e9ka\u0159i p\u0159isp\u00edvaj\u00ed do <a href=\"https:\/\/en.wikipedia.org\/wiki\/Shared_decision-making_in_medicine\">procesu l\u00e9\u010debn\u00e9ho rozhodov\u00e1n\u00ed<\/a>, p\u0159i\u010dem\u017e se pacient\u016fm dost\u00e1v\u00e1 podpory, aby zv\u00e1\u017eili mo\u017enosti a informovan\u011b si vytvo\u0159ili preference a p\u0159edstavu o po\u017eadovan\u00fdch v\u00fdsledc\u00edch.<\/p>\n<p>Spole\u010dn\u00e9 rozhodov\u00e1n\u00ed se v oblasti <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17485749\">zdravotn\u00ed p\u00e9\u010de<\/a> dost\u00e1v\u00e1 st\u00e1le v\u00edce do pop\u0159ed\u00ed, a mnoho profesion\u00e1ln\u00edch poskytovatel\u016f zdravotn\u00ed p\u00e9\u010de se sna\u017e\u00ed co nejv\u00edce postupovat <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20947577\">s\u00a0ohledem na pacienta<\/a>; t\u00edm sp\u00ed\u0161e, kdy\u017e tento komunika\u010dn\u00ed p\u0159\u00edstup m\u016f\u017ee zlep\u0161it adherenci k\u00a0l\u00e9\u010db\u011b a d\u016fv\u011bru.<\/p>\n<p>Ve sv\u00e9 podstat\u011b je tedy spole\u010dn\u00e9 rozhodov\u00e1n\u00ed zalo\u017eeno na porozum\u011bn\u00ed a <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15150208\">respektu<\/a>.<\/p>\n<p>L\u00e9ka\u0159 a pacient jsou v\u00a0tomto procesu rovnocenn\u00fdmi partnery a t\u00edm, \u017ee sd\u00edl\u00ed svou expert\u00edzu a zku\u0161enosti, oba p\u0159isp\u00edvaj\u00ed k rozhodnut\u00ed. T\u00edm je my\u0161leno, \u017ee poskytovatel p\u00e9\u010de p\u0159isp\u00edv\u00e1 l\u00e9ka\u0159sk\u00fdmi znalostmi a svou kvalifikac\u00ed a pacient p\u0159in\u00e1\u0161\u00ed znalosti a zku\u0161enosti z\u00a0oblasti sv\u00e9ho osobn\u00edho \u017eivota.<\/p>\n<p>V\u00a0teorii je to v\u0161ak jednodu\u0161\u0161\u00ed ne\u017e v\u00a0praxi.<\/p>\n<p>Zat\u00edmco mnoho odborn\u00edk\u016f z\u00a0oblasti zdravotn\u00ed p\u00e9\u010de tvrd\u00ed, \u017ee u\u017e \u201eto d\u011bl\u00e1\u201c, \u00fadaje z\u00a0<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20393104\">pr\u016fzkum\u016f zku\u0161enost\u00ed pacient\u016f<\/a> nazna\u010duj\u00ed, \u017ee <a href=\"https:\/\/www.bmj.com\/content\/341\/bmj.c4989.long\">tomu tak v\u0161eobecn\u011b nen\u00ed<\/a>.<\/p>\n<p><em>Dermatolo\u017eka, se kterou jsem mluvila, m\u011bla ve zvyku nerozhodovat za pacienty, ale rozhodovat spolu s\u00a0nimi o tom, jak\u00e1 l\u00e9\u010dba by jim sed\u011bla nejl\u00e9pe. Proto v\u016fbec nech\u00e1pala, pro\u010d jej\u00ed pacienti nebyli motivovan\u00ed br\u00e1t pr\u00e1\u0161ky p\u0159esn\u011b podle p\u0159edpisu, a to ani p\u0159esto, \u017ee l\u00e9\u010dbu zalo\u017eila na osobn\u00edch preferenc\u00edch ka\u017ed\u00e9ho z nich. Kdy\u017e jsem se j\u00ed zeptala, jak vypad\u00e1 proces spole\u010dn\u00e9ho rozhodov\u00e1n\u00ed v\u00a0jej\u00ed ka\u017edodenn\u00ed klinick\u00e9 praxi, popsala, \u017ee jakmile k\u00a0n\u00ed pacient vkro\u010d\u00ed, informuje ho, \u017ee spolu na konci konzultace rozhodnou o form\u011b l\u00e9\u010dby, a proto spolu budou prob\u00edrat r\u016fzn\u00e9 mo\u017enosti.<\/em><\/p>\n<p>Pacienti pot\u0159ebuj\u00ed leckdy \u010das, aby nov\u00e9 informace vst\u0159ebali a zv\u00e1\u017eili sv\u00e9 osobn\u00ed preference, ne\u017e se zav\u00e1\u017eou k\u00a0nov\u00e9mu rozhodnut\u00ed. To plat\u00ed obzvl\u00e1\u0161t\u011b tehdy, je-li jejich budouc\u00ed zdravotn\u00ed stav nejist\u00fd a mus\u00ed-li uva\u017eovat o v\u00fdsledc\u00edch, se kter\u00fdmi dosud nem\u011bli \u017e\u00e1dnou <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/184205\">zku\u0161enost<\/a>.<\/p>\n<p>P\u0159i zahrnut\u00ed do procesu rozhodov\u00e1n\u00ed tak mohou b\u00fdt n\u011bkte\u0159\u00ed pacienti p\u0159ekvapen\u00ed, znepokojen\u00ed a nejist\u00ed, kter\u00e1 z\u00a0nab\u00edzen\u00fdch mo\u017enost\u00ed by mohla b\u00fdt nejlep\u0161\u00ed.<\/p>\n<p><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0738399105000546?via%3Dihub\">Efektivitu<\/a> spole\u010dn\u00e9ho rozhodov\u00e1n\u00ed tedy m\u016f\u017ee zlep\u0161it <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3010418\/\">v\u010dasn\u00e9<\/a> informov\u00e1n\u00ed pacient\u016f o r\u016fzn\u00fdch zp\u016fsobech l\u00e9\u010dby a o tom, \u017ee se od nich o\u010dek\u00e1v\u00e1 \u00fa\u010dast na spole\u010dn\u00e9m rozhodov\u00e1n\u00ed.<\/p>\n<p><em>Aby sv\u00fdm pacient\u016fm pomohla v\u00a0rozhodov\u00e1n\u00ed, kter\u00e1 forma l\u00e9\u010dby by nejl\u00e9pe odpov\u00eddala jejich osobn\u00edm po\u017eadavk\u016fm, pou\u017e\u00edvala dermatolo\u017eka procenta (nap\u0159. zlep\u0161en\u00ed o 10% nebo 20%). Do procesu rozhodov\u00e1n\u00ed pacienty aktivn\u011b zahrnula tak, \u017ee pot\u00e9, co si vybrali procento, kter\u00e9 by jim nejl\u00e9pe vyhovovalo, vysv\u011btlila jim, jakou l\u00e9\u010dbou a medikac\u00ed toho lze dos\u00e1hnout. P\u0159es to v\u0161echno m\u011bli jej\u00ed pacienti st\u00e1le probl\u00e9my l\u00e9\u010dbu dodr\u017eovat, a to i kdy\u017e si sami vybrali mo\u017enost, kter\u00e1 nejl\u00e9pe nasedala na jejich osobn\u00ed situaci. Zeptala jsem se j\u00ed, zda si mysl\u00ed, \u017ee jej\u00ed pacienti maj\u00ed p\u0159edstavu, jak on\u011bch 20%, 30% atd. vypad\u00e1, a jestli by i oni sami pou\u017eili stejn\u00fdch v\u00fdraz\u016f, pokud by m\u011bli popsat vytou\u017eenou \u00falevu od symptom\u016f n\u011bkomu jin\u00e9mu.<\/em><\/p>\n<p>\u010c\u00e1st cel\u00e9ho <a href=\"http:\/\/www.theguardian.com\/healthcare-network\/2014\/jul\/01\/impact-medical-jargon-patient-centred-care\">probl\u00e9mu<\/a> le\u017e\u00ed v\u00a0neodmysliteln\u00e9 tendenci odborn\u00edk\u016f (zdravotn\u00edk\u016f) <a href=\"http:\/\/www.sciencedirect.com\/science\/article\/pii\/S0020748915000619?via%3Dihub\">pou\u017e\u00edvat jazyk<\/a>, kter\u00fd je srozumiteln\u00fd hlavn\u011b koleg\u016fm z\u00a0oboru. L\u00e9ka\u0159sk\u00fd jazyk m\u016f\u017ee b\u00fdt velmi slo\u017eit\u00fd, a jak ukazuj\u00ed v\u00fdzkumy, <a href=\"https:\/\/www.telegraph.co.uk\/news\/health\/news\/10909421\/Nearly-half-of-patients-do-not-understand-medical-language.html\">t\u00e9m\u011b\u0159 polovina pacient\u016f<\/a> m\u00e1 obt\u00ed\u017ee mu <a href=\"http:\/\/www.bmj.com\/content\/320\/7233\/484?fromsource=nelm;%20www.tandfonline.com\/doi\/abs\/10.3109\/09638288.2013.793409\">porozum\u011bt<\/a>.<\/p>\n<p>\u017dargon a abstraktn\u00ed pojmy um\u00ed vy\u010darovat \u0161irokou \u0159adu interpretac\u00ed, kter\u00e9 se v\u00fdrazn\u011b vzdaluj\u00ed tomu, co m\u011bl zdravotn\u00edk p\u016fvodn\u011b na mysli. Velk\u00e9 mno\u017estv\u00ed d\u016fkaz\u016f nav\u00edc sv\u011bd\u010d\u00ed o tom, \u017ee i u sebehodnot\u00edc\u00edch polo\u017eek m\u016f\u017ee doch\u00e1zet k\u00a0v\u00fdznamn\u00e9mu zkreslen\u00ed, pop\u0159\u00edpad\u011b v\u016fbec <a href=\"https:\/\/www.telegraph.co.uk\/news\/health\/news\/10909421\/Nearly-half-of-patients-do-not-understand-medical-language.html\">nem\u011b\u0159\u00ed to, k\u00a0\u010demu byly ur\u010deny<\/a>.<\/p>\n<p>V\u00a0dotazn\u00edku mohou lid\u00e9 za\u0161krtnout, \u017ee nejsou schopni uj\u00edt 500 metr\u016f, ale v\u00a0navazuj\u00edc\u00edm rozhovoru prozrad\u00ed, \u017ee \u010dasto chod\u00ed po obchodn\u00edm centru i d\u00e9le ne\u017e hodinu.<\/p>\n<p><em>Dermatolo\u017ece jsem \u0159ekla, \u017ee pokud bych byla na m\u00edst\u011b pacienta, p\u0159\u00e1la bych si jako c\u00edl l\u00e9\u010dby n\u011bco jako b\u00fdt schopn\u00e1 si s\u00a0n\u011bk\u00fdm pot\u0159\u00e1st rukou, nebo nosit triko s\u00a0kr\u00e1tk\u00fdm ruk\u00e1vem. Z\u00edrala na m\u011b a za\u010dala se sm\u00e1t, \u017ee m\u00e1m samoz\u0159ejm\u011b pravdu, ale sama by na to nikdy nep\u0159i\u0161la, a \u017ee si tak\u00e9 dosud neuv\u011bdomila, \u017ee by mohlo b\u00fdt efektivn\u011bj\u0161\u00ed p\u0159edem informovat pacienta o procesu spole\u010dn\u00e9ho rozhodov\u00e1n\u00ed.<\/em><\/p>\n<p>Je tedy z\u00e1sadn\u011b d\u016fle\u017eit\u00e9, aby zdravotn\u00edci m\u011bli skute\u010dn\u00fd z\u00e1jem o pacienty, kter\u00e9 l\u00e9\u010d\u00ed, a rozum\u011bli jejich ch\u00e1p\u00e1n\u00ed nemoci. A to se net\u00fdk\u00e1 pouze pacient\u016f s\u00a0omezenou (zdravotn\u00ed) gramotnost\u00ed, proto\u017ee v\u0161ichni pacienti mohou m\u00edt prosp\u011bch z\u00a0jasn\u00e9 a v\u00fdsti\u017en\u00e9 informace opro\u0161t\u011bn\u00e9 od l\u00e9ka\u0159sk\u00e9 hant\u00fdrky. Proto je nesm\u00edrn\u011b d\u016fle\u017eit\u00e9, aby l\u00e9ka\u0159i komunikovali v\u00a0jazyce, na kter\u00fd jsou pacienti zvykl\u00ed, a aby se vyjad\u0159ovali slovy, kter\u00e1 by pou\u017eili sami pacienti.<\/p>\n<p>Praktick\u00e1 doporu\u010den\u00ed:<\/p>\n<ul>\n<li>V\u011bt\u0161\u00edm zapojen\u00edm pacienta do proces\u016f rozhodov\u00e1n\u00ed m\u016f\u017eeme zlep\u0161it adherenci k\u00a0l\u00e9\u010debn\u00e9mu re\u017eimu a v\u00fdsledky l\u00e9\u010dby.<\/li>\n<li>V\u017edy se sna\u017ete vyjad\u0159ovat se slovy, kter\u00e1 by pou\u017eil s\u00e1m pacient (tj. vyh\u00fdbejte se l\u00e9ka\u0159sk\u00e9 hant\u00fdrce)<\/li>\n<li>M\u011bjte na pam\u011bti, \u017ee m\u016f\u017ee doj\u00edt k\u00a0v\u00fdznamn\u00e9mu nedorozum\u011bn\u00ed, i kdy\u017e m\u00e1te pocit, \u017ee mluv\u00edte zcela jasn\u011b.
